This worksheet should ONLY be completed by students serious about having a Peer Mentor. Completing this form is an indication that you are agreeing to be matched and will follow the program requirements for mentees. Program requirements are listed on the Peer Mentor Request Form. Also, the form allows submissions to stand as authorization for the Peer Mentor Coordinator to share the mentee’s email address with the matched Peer Mentor. Give careful consideration to completing this step. Mentees would then attend a Mentee Orientation for more information on expectations, make a commitment, and meet their Peer Mentor.
